The Accident, Exposure, Injury, Illness & Property Loss packets will no longer be furnished within each station's safety binder. Access via the server on the "Y" drive under "Safety Committee", or the website under "Resources"; "Forms and Docs". Print entire packet, complete only those forms applicable to your needs, and return entire packet to me in a timely manner. Don't forget to notify the proper workers comp agency, if needed. Each binder will include these same instructions. The affected member's immediate supervisor is responsible for filing paperwork.

Also, It's under what the title of the PDF is named, Employee/Volunteer Property Loss Packet. Not sure if that's gonna be to hard to find and needs to be located under the following titles as well to make it easier to find when incident happens:
