To all employees:
When returning to regular shift work following medical leave or light-duty assignment, you must forward your BaptistWorx "Return to Duty" form to BC Mattingly for proper documentation.

If under care or off do to a non work related injury/illness and your private doctor clears you to return to work there should be no reason to also be cleared by Baptist Work doctor. The Baptist Work doctors would not know what care you have been under. Plus seems like a wasted expense for the department to incur.

Was referring to work-related illness/injuries only. If you were initially cared for/treated at BaptistWorx, then forward "Return to Duty" paperwork to AC Mattingly before returning to shift. Non-work related illness/injury "Return to Duty" forms must be run through AC Mattingly, as well. Bottom line; no one can return to shift without presenting an "all clear". BaptistWorx does not need to authorize off-duty illness/injury "Return to Duty" paperwork.
